WHAT IS CASE MANAGEMENT?

To assist you in overcoming your individual setbacks and persistent challenges to recovery, we provide case management. This is the coordination of community-based services headed by a professional and supported by a team of our staff. We seek to reduce hospitalizations and provide overall support toward your recovery. We assess your physical, psychological, and social needs in a holistic way to create a plan that actually reduces overall costs. This plan and coordination includes our mental health services, and also may include primary healthcare, housing, employment, education, transportation, community participation, and social relationships. Our exceptional case managers work to ensure that care is provided in the most comprehensive and cost-effective way for you and your insurance providers.

THE BENEFITS OF CASE MANAGEMENT

Our case managers can be positive forces for good on your road to recovery. They advocate for your needed services, coordinate care, ensure care continuity, and help manage financial needs. They can offer help with issues based on medication and treatment adherence, readmissions, and communication with the clinical team and various providers, among others.

Oftentimes case managers are more aware of the services, providers, and resources available in our community and can maximize these for your progress. They act as your personal supporter during the assessment, planning, treatment, and post-treatment phases to meet your individual health needs.

PAYMENT

We are committed to serving all people regardless of their race, disability, sex, sexual orientation, religion or inability to pay. We offer discounted fees for patients who qualify based upon their family size and income. We accept Medicaid, Medicare, state funding for substance abuse disorders, BPA, Crime Victim’s health insurance, and all major private health insurance. No one will be denied access to services due to an inability to pay.
